Patients
- Please contact us to schedule an appointment with the following information to setup your patient account: name, address, date of birth, social security number, phone number(s), email, and dental insurance information.
- Let us know immediately if you are having any pain or dental issues so that we can address those promptly and schedule as necessary.
- New Patients: We will contact prior dentist office to have dental history (x-rays, panorex, cleaning info, perio charting, notes, etc) emailed to us at info@stevedouglasddspc.com prior to first appointment. Please let them know that we will be contacting them.
- Forms will be sent electronically via mail or text 1-2 days prior to your appointment.
- Bring dental insurance card and driver's license for us to copy.
