• Cigna PPO & Medicare Advantage (PPO Advantage get disc and coverage, but owe difference between the PPO and PPO Advantage plans)

****Some listed above are THRU one of our contracted insurances and there are many other “underlying” insurances that we may be participating with THRU these insurances as well.  

However, if their relationship changes we may not be notified until after your appointment that you are now OUT-of-network.  We can send your claim for processing, but your plans terms may be different %/frequency/amount paying as OUT-of-network.  Please be aware of whether your plan has BOTH IN and OUT-of-network coverage and what those terms are if yours is THRU another one of our contracted insurances.

Ultimately it is your responsibility to know what your plan covers

If your insurance changes, please contact us ASAP (48 hours prior to upcoming appointments preferred) so that we can verify and update on your account.
